How to Get Care
There are two possible paths based on where you are in the VA process:
Track 1: Already Approved for VA Services
Contact the Social Worker at the VA Clinic where your VA physician is located.
Let them know you want to switch your HHA (Homemaker & Home Health Aide) services to Home Instead.
The Social Worker will initiate the transfer process.
Once the Minneapolis VA office approves the change, Home Instead will contact you to schedule an RN assessment and start services.
Track 2: New to VA Services
Find the closest VA Clinic to your location.
Contact them to establish care with a VA physician.
At your appointment, tell the physician you’re requesting Homemaker & Home Health Aide services through Home Instead.
The Social Worker will begin the approval process.
After the Minneapolis VA office approves, Home Instead will reach out to schedule an RN assessment and begin care.
