About Nurse Directed Care

Our specially trained Care Pros can make life easier by providing highly personalized care for common health conditions that affect families, such as dementia, diabetes, Parkinson’s disease, heart failure, COPD, depression, arthritis and more. Nurse directed home care are services performed or delegated by a nurse and may include:

Nursing Services

  • Medication management including the administration of intramuscular injections into the skin.
  • Provide assistance with home dialysis.
  • Assist with diabetic wound care on client’s feet.

Medication Management and Administration

  • Manage medication by crushing or splitting medications as well as drawing dosage for oral liquid medications, as prescribed.
  • Administer medications by placing medication in a senior’s mouth if he or she is unable to do so.

Vital Sign Tracking and Interpreting

  • Take a senior’s blood pressure and temperature.
  • Interpret vital sign outcomes and respond to thresholds.

Wound Care Support

  • Provide wound care with simple dressings.
  • Provide bathing assistance when a senior has a wound or dressing.
  • Assist with skin care when a senior has an open wound.

Diabetes/Glucose Testing Management

  • Monitor glucose (i.e. Accuchecks) to determine blood sugar reading.
  • Administer insulin and record and document findings.

Foot and Nail Care

  • Apply cream to a senior’s feet.
  • Provide care to wounds on feet.
  • Nail care on hands and feet.

Catheter Care

  • Emptying a catheter drainage bag.
  • Remove or replace a pre-sized wafer around a stoma.

Oxygen Therapy

  • Adjust a flowmeter to the prescribed setting.
  • Monitor the oxygen saturation levels.

Feeding Tube

  • Turn on and off a feeding tube machine.
  • Flush feeding tubes.
  • Change dressing around the feeding tube.

We offer a variety of personalized services, including personal care, Alzheimer’s and dementia support, hospice assistance, chronic illness care, household help, transportation, companionship, and nurse-directed care.

Do you have minimum hours required for home care services?

Yes, typically we require a minimum number of hours per visit or per week to ensure consistency and quality of care. We will discuss your specific needs during our initial assessment.

Are your caregivers trained and background checked?

Absolutely. All our care pros undergo rigorous training, comprehensive background checks, drug testing, motor vehicle checks, and ongoing professional education to ensure your loved one receives safe, compassionate care.

Will the same caregiver visit each time?

We strive to provide consistent caregiver assignments to build strong, trusting relationships. While schedules can occasionally vary, our goal is to keep changes minimal and inform you in advance

Can you help with medications and medical care?

Our caregivers can provide medication reminders and assistance with medication administration as guided by our nurse-directed care plans. However, they cannot prescribe or dispense medications directly.

How soon can you begin providing services?

We aim to begin care promptly. Often, we can initiate services within 24 to 48 hours following the initial assessment, depending on caregiver availability, client location and client-specific requirements.

Do you accept insurance or Medicare/Medicaid?

We will help you review options and can accept various long-term care insurance plans, as well as private pay. Currently, Medicare does not typically cover non-medical home care, but some Medicaid programs may provide coverage depending on eligibility. 

Can we adjust the level of care if our needs change?

Yes, our nurse-directed care plans are flexible and regularly reviewed. We can easily adjust the level of care to meet changing health conditions or personal preferences.
